Our Community partners are Canadians who are familiar with the Canadian Healthcare system. Care partners form long-term relationships with people seeking support in their journey through the healthcare system. A care partner can help you navigate the system and get the care you need.

What can my community partner do for me?
Community partners can help you set up appointments, register with a family physician and even accompany you to an appointment. Community partners help you by taking notes during appointments, advocating for you and helping you communicate with your care team.

How to get a community partner?
You can request a community partner by contacting us via email or using the form located below. Once your application has been received you will be connected with a community partner. You community partner will reach out to you and introduce themselves and what they can do to best support you.
